Permanente:
No physical exam required.
For Blue Cross:
No physical exam required.
For Blue Shield:
No physical exam required. However,
if the applicant is a US citizen
and has been out of the country
for some time, underwriting might request
for a physical exam conducted by
a California physician.
For Kaiser
Permanente, Blue Cross and Blue
Shield:
Applicants will be declined since
pregnancy is considered a pre-existing
condition.

Permanente and Blue Cross:
It is required that you give either
your Social Security Number or
Tax ID Number when applying. For
newborns, they may submit their
Social Security Number upon receipt.
For Blue Shield:
You are not required to give your
Social Security Number. However,
you have to answer the question
regarding residency found on the
first page of the application form.
For Kaiser
Permanente:
It could take 5-7 business days
for your application to be completely
processed.
For Blue Cross and Blue
Shield :
Usual processing could take 7-10
business days. However, processing
may take longer if upon review
of your medical history, additional
medical information is needed from
your previous physician.
For Kaiser Permanente:
You will receive an email regarding
the status of your application
in 5-7 business days. Kaiser
Permanente will also contact
you if they need some more
information to help them evaluate
your application.
For Blue Cross:

For Kaiser
Permanente:
Applications submitted before the
23rd of the month will be given
an effective date of the first
day of the same month. Applications
submitted after that will be given
an effective date of the first
day of the following month.
For Blue Cross and Blue
Shield :
You may choose the date when you
would want your coverage to be
effective. You may choose the date
beginning from the day after the
signature date but not more than
60 days after. They may also assign
your effective date which is usually
the day after they approved your
application.
For Kaiser
Permanente:
You may choose to include dental
coverage upon application. If not,
you may add it within the first
30 days of your coverage. If it’s
been more than 30 days, you’ll
have to wait until December for
open enrollment to include it.
For Blue Cross and Blue
Shield :
You may choose to include dental
application upon enrollment or
have it added to your coverage
anytime.
For Kaiser Permanente,
Blue Cross and Blue Shield:
Your monthly rate will change
if:
- You add a dependent
- You move to a different age
group
- You move to a different rate
area
- During annual rate adjustments
- You switch plans
For Kaiser
Permanente:
Your rates will be the same until
December 31st. Rates may change
if you move to a different age
group, add a dependent or move
to a different rate area.
For Blue Cross:
Your rates are guaranteed for 6
months from the day your coverage
started. After the guarantee period,
your rates may be adjusted by the
company. However, your rate will
change due to the annual rate adjustment,
if you move to a different age
group, add a dependent, move to
a different rate area or change
plans.
For Blue Shield:
Your rates will remain the same
during the 6 months guarantee period.
Still, your rate will change due
to the annual rate adjustment,
if you move to a different age
group, add a dependent, move to
a different rate area or change
plans.
*Rates for Blue Shield
applications approved by March
thru December 2008 are guaranteed
for 12 months.
For Kaiser
Permanente, Blue Cross and Blue
Shield:
Applicants who were declined coverage
when they applied for regular individual
or family plans could apply for
HIPAA (Health Insurance Portability
and Accountability Act of 1996).
HIPAA is more expensive but it
is a good option to consider especially
if you do not qualify for regular
plans and have fully used COBRA.
